    * There is a current waitlist for Medicaid clients. Please fill out the form below to join the waitlist. Foster care and Adoptive children, families and young adults will be placed on a priority list.

    We are also happy to provide you with a supper bill if you would like to seek reimbursement independently or utilize out of network benefits from other insurance carriers.
